If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Photo ID (Parent/Guardian)
- Complete immunization records
- Medical records from your previous pediatrician
- List of all current medications
Well Checks: We require your child to be seen at our office for all well checks. Our schedule of well checks is as follows:
- 2 to 4 days
- 1 week
- 2 weeks
- 1 month
- 2 months
- 4 months
- 6 months
- 9 months
- 12 months
- 15 months
- 18 months
- 24 months
- 30 months
- 3 years
- And once every year thereafter for an annual health supervision visit that includes a physical exam as well as a developmental, behavioral, and learning assessment.
Technology: Our practice prides itself on efficiency through the use of technology. We offer MyChart to all our patients.
Office Hours: We are open Monday-Friday 8:00am-6:00pm and Saturdays 9-2. We are closed on major holidays.
After Hours: If you need to speak with a nurse outside of our normal working hours, we encourage you to contact the Children's Medical Center Nurse Advice Line. This can be done by calling our office and following the prompts to be connected to the nurse line. There is no charge for this service. If you are experiencing a medical emergency, please call 911.
Prescription Refills: All refill requests must be made during office hours. Please give at least 3 business days for refills to be completed or5- 7 business days for controlled medications (such as ADHD medications) to be refilled.
Medical Forms: We will need 7 business days to complete all the medical forms. We strive to complete these forms sooner, however that is not always the case. There is a fee for completing all forms, unless the parent provides the needed form at the child’s visit.
Insurance: Please make sure we take your specific insurance plan. To do this, call our office and/or check with your specific insurance company. We will bill the insurance companies with whom we are contracted. Payment is expected at the time of service for any copays and deductibles not covered by your plan. We accept cash and credit cards.
We offer a discounted self-pay rate and are in-network with many insurance plans. Listed below are some of our in-network plans. If your plan is not listed, please contact the office.

Billing and Fees: It is up to you to understand how your insurance works, including deductibles and co-insurance, and to provide up-to-date insurance information at every visit. Our office fees are as follows:
- LATE: If you are more than 15 minutes late, you may be required to reschedule your appointment.
- NO SHOW: No show appointments will be charged $50 for each missed appointment. Your child(ren) will be dismissed after the 3rd missed appointment for your family.
- CANCELLATIONS: A $50 fee will be charged for each appointment that is cancelled within 24 hours of the appointment.
